Reframing body 'weight'

How I use GLPs and Mounjaro

It is not just about weight

GLP medications have become highly visible in the media often framed as “weight loss drugs” or quick fixes. But in clinical practice, the reality is far more nuanced.

For many people, particularly women in perimenopause and midlife, weight gain is not simply about eating too much or exercising too little. Hormonal changes, stress, poor sleep, insulin resistance, changing body composition, chronic inflammation, emotional load, medications, trauma, and life-stage pressures can all profoundly affect appetite regulation, energy balance, and metabolic health. Often, these changes occur alongside a shifting sense of identity and confidence, leaving people feeling frustrated, ashamed, and unsupported.

In our practice, GLP medications are never viewed as a stand-alone solution or a cosmetic tool. Instead, they may be used as part of a broader, evidence-informed approach to improving health, function, and quality of life. For the right person, they can help reduce food noise, improve metabolic flexibility, support insulin resistance, and create enough physical and emotional capacity for sustainable change.

The goal is not rapid weight loss at any cost.

The goal is better health outcomes:

  • improved strength and mobility
  • healthier ageing
  • reduced metabolic risk
  • improved energy and sleep
  • greater confidence and self-efficacy
  • and a renewed ability to engage in meaningful lifestyle change

This is why nutrition, resistance training, sleep, nervous system regulation, and psychological wellbeing remain central to treatment.

Medication may provide support, but long-term health comes from rebuilding strength, resilience, and trust in the body.

Weight reduction is more than just willpower.

GLP medications do far more than just reduce calorie intake. They influence appetite signalling, insulin regulation, gastric emptying, reward pathways, inflammation, satiety, and the constant “food noise” many people experience. For some patients, they reduce the relentless mental load around food for the first time in years.

Reflecting on Overweight and Obesity

Understanding what overweight or obesity means to you is a crucial first step in your health journey. It’s not just about numbers on a scale; it’s about how you feel in your body and the impact it has on your daily life. At FemaleGP, we encourage you to think deeply about your personal experiences and feelings regarding weight, as this can shape your goals and aspirations.

What are your goals?

Setting clear goals is essential for achieving the health outcomes you desire. Whether you aim to improve your overall health, enhance your mobility, or simply feel more confident in your skin, defining your objectives will guide your journey. Consider what success looks like for you and how it aligns with your lifestyle.

What do you want your life to look or feel like?

Visualising your ideal life can be a powerful motivator. Do you see yourself with more energy to engage in activities you love? Or perhaps you envision a life free from pain and discomfort? Whatever your vision, it’s important to articulate it, as this will help you stay focused and committed to your health goals.

Is your goal: Health, Mobility, or Confidence?

Your goals may vary widely, from improving your health and mobility to boosting your confidence. Each goal requires a tailored approach, and at FemaleGP, we are here to support you in identifying the best strategies to achieve them. Think about what matters most to you and how we can assist you in reaching those milestones.

What about Diabetes Prevention or Fertility?

For some, the focus may be on preventing diabetes or enhancing fertility. These are significant health concerns that can be influenced by weight and lifestyle choices. If these resonate with you, we can provide guidance and resources to help you navigate these challenges effectively, ensuring you have the support you need.

Understanding Mounjaro & GLP-1 Medications

What are GLP-1 medications?

GLP-1 (Glucagon-Like Peptide-1) medications mimic a natural gut hormone that:

  • Helps the body release insulin after eating
  • Slows stomach emptying → helping you feel fuller for longer
  • Reduces appetite and food cravings
  • Supports more stable blood sugar levels

These medications can create “brain space” around food and appetite, helping people make more sustainable wellbeing, movement, nutrition, and lifestyle changes.

Benefits of GLP-1 Medications

GLP-1 medications like Mounjaro can aid in weight loss, improve blood sugar levels, and reduce the risk of cardiovascular diseases. They are particularly beneficial for women navigating hormonal changes.

Approximate Costs in New Zealand (correct as of 05/26)

Private/self-funded medications can be expensive.  Not all GPs/Specialists are happy to prescribe unfunded medications because they may not have had training in it.

Private scripts are also more expensive (but often these are included in the cost of private medications

Switching between GLP-1 Medications

General approach

  • Start with an equivalent dose or one dose lower
  • Use a conservative approach to reduce gastrointestinal side effects
  • Start the new medication when the next dose of the previous medication was due

If switching because of side effects

  • Wait until symptoms improve
  • Start at the lowest possible dose
  • Consider slower dose escalation

Dose titration should be based on:

  • Tolerability
  • Weight response
  • Blood sugar response

Understanding Side Effects of GLP-1 Medications

GLP-1 medications are designed to help manage weight and improve metabolic health by slowing stomach emptying and altering appetite signals. While these medications can be effective, they may also lead to side effects as your body adjusts to the changes. Over time, many individuals find that these side effects diminish as their gut and brain adapt, and as their dosing stabilises.
  • What Common Side Effects Can I Expect?

Common side effects of GLP-1 medications are typically mild to moderate and often improve with time. These may include gastrointestinal issues like nausea, vomiting, and bloating, as well as general symptoms such as fatigue and dizziness. Injection site reactions like redness and swelling may also occur.
  • How Can I Manage Low Blood Sugar?

If you're using GLP-1 medications like Mounjaro alongside insulin or sulfonylureas, you may be at a higher risk for low blood sugar (hypoglycaemia). Mild symptoms include sweating and hunger, while severe symptoms can lead to confusion or seizures. It's important to consume fast-acting carbohydrates if you experience hypoglycaemia and seek medical advice if symptoms are severe or recurrent.
  • When Should I Seek Medical Attention?

Certain serious side effects require urgent medical attention. Symptoms of acute pancreatitis, gallbladder problems, allergic reactions, or severe gut slowing should not be ignored. If you experience severe abdominal pain, persistent vomiting, or difficulty breathing, seek help immediately.
  • Will I Need to Stay on GLP-1 Medications Long-Term?

The duration of GLP-1 medication use varies by individual. For some, these medications serve as short-term tools, while for others, they may be part of a long-term management plan for obesity. Discussing your long-term goals and strategies with your healthcare provider is essential for effective management.
  • What Should I Consider When Starting Treatment?

When beginning treatment with GLP-1 medications, it's crucial to have a comprehensive plan. This should include discussions about nutrition, exercise, emotional wellbeing, and stress management to support your overall health and weight management journey.

Mounjaro Checklist – What You Need

Before starting your Mounjaro treatment, it's essential to have everything organised. This checklist will help ensure you have all the necessary equipment and understand the best practices for safe and effective use.
  • What equipment do I need?

You'll need a Mounjaro KwikPen, a new pen needle for every injection, a sharps bin for safe disposal, alcohol wipes if you prefer, and some fridge storage space. Don't forget to set reminders for needle changes!
  • Why is using a new needle important?

Using a new needle each time helps reduce the risk of infection, needle blunting, pain, medication contamination, and incorrect dosing. Always remove the needle after each injection and never leave it attached between doses.
  • How should I dispose of used needles?

Used needles should be placed in an approved sharps container. Be aware that not all pharmacies participate in sharps disposal programmes, so check if your pharmacy is part of a local Sharps Management Plan.
  • How should I store Mounjaro?

Mounjaro should be stored in the fridge at 2°C to 8°C. If refrigeration isn't possible, it can be kept at room temperature below 30°C for up to 30 days. Always keep the pen in its original packaging to protect it from light.
  • What if the pen freezes?

Do not freeze the KwikPen. If it accidentally freezes, it should be discarded. Proper storage is crucial for maintaining the medication's effectiveness.

Dose Escalation & Click Counting

Practical Tips for Mounjaro Use

Avoid leaving your Mounjaro pen in hot cars or direct sunlight. When travelling, consider using cool storage packs to maintain the right temperature. Proper handling and storage are key to ensuring your medication remains effective.
